New York City Tour 2012



Please download and print the following five forms. The forms should be completed and submitted ASAP — by the March 26 meeting at the latest. PBA will provide a notary at the March 26 meeting.



Payment Information

Educational Tours, Inc.Payments will be made through Educational Tours, Inc. through their online payment system; instructions were sent out with the tour commitment form — don’t worry about the initial $100 payment listed for July 1. The first payment of $300 was due on Sept. 15.  The final tour payment schedule is below.

Families with need may apply for partial scholarships—please use the general scholarship application form and turn it in as soon as possible.  PBA has a limited budget for scholarships, and the scholarship committee will be meeting soon to decide how to allocate assistance.
